Government Reporting

Ascensus Consulting, an independent third-party administrator, assists clients in reporting certain required information to government entities for tax qualified defined contribution, defined benefit, and cash balance plans.

Employers are required to file an annual return, or report, with the Federal government called a Department of Labor Form 5500 Annual Return/Report of Employee Benefit Plan. The Form 5500 is designed to disclose information about the plan and its operation to the Department of Labor, the plan’s participants, and to the public. The Internal Revenue Service also requires employers to submit completed forms annually.

Ascensus Consulting facilitates the government forms filing process for its clients by providing the following signature-ready forms:
  • The Department of Labor requires employers to electronically file the Form 5500 or Form 5500-SF schedule by using an on-line filing system.
  • The Internal Revenue Services requires that employers annually file the Form 5500-EZ (for 1-participant plans) or the Form 8955-SSA (to report terminated participants due a benefit).
